Abstract
Cilj istraživanja: Primarni cilj ovog istraživanja je bio utvrditi prevalenciju sekundarne antimikrobne rezistencije bakterije H. pylori u KBC Split od 2018. do 2020. godine. Sekundarni ciljevi ovog istraživanja su utvrditi prevalenciju sekundarne antimikrobne rezistencije H. pylori u KBC Split od 2018. do 2020. godine prema spolu i dobnim skupinama. Ispitanici i metode: U ovo retrospektivno kliničko istraživanje (studijski period 2018. − 2020.) uključeno je 58 pacijenata (41 žena i 17 muškaraca) s dokazanom infekcijom H. pylori u kojih je utvrđena sekundarna antimikrobna rezistencija. Rezultati: Prevalencija sekundarne antimikrobne rezistencije H. pylori u KBC Split od 2018 do 2020. godine na pojedine antibiotike poklapa se s brojnim istraživanjima koja su provedena u Europi i Hrvatskoj, tj. najveća rezistencija zabilježena je na klaritromicin i levofloksacin. Sekundarna antimikrobna rezistencija H. pylori u KBC Split od 2018 do 2020. godine bila je bitno veća u žena nego u muškaraca (p=0,0022). Omjer je iznosio 71% prema 21%. Najveća učestalost sekundarne antimikrobne rezistencije H. pylori s obzirom na dob zabilježena je u dobnoj skupini od 60 do 85 godina, a iznosila je 50% (p<0,0001). Najmanja rezistencija je zabilježena na amoksicilin, a najveća na klaritromicin (p<0,0001). Starenje se pokazalo kao zaštitni čimbenik kad je riječ o rezistenciji na tetraciklin (p=0,0013). Zaključak: Istraživanje o sekundarnoj rezistenciji bakterije H. pylori na uobičajene antibiotike u KBC Split u trogodišnjem razdoblju (2018. do 2020.) potvrđuje opravdanost strategije liječenja utemeljenog na rezultatima kultura jer nakon prethodnih neuspješnih (empirijskih) antimikrobnih terapija, samo ciljana antimikrobna terapija dovodi do eradikacije bakterije H. pylori.
Abstract (english)
Goal: The primary goal of this study was to determine the prevalence of secondary antimicrobial resistance of bacteria H. pylori in KBC Split from 2018 to 2020. The secondary goals of this study were to determine the prevalence of secondary antimicrobial resistance of H. pylori in KBC Split from 2018 to 2020 by sex and age groups. Materials and methods: This retrospective clinical study (study period 2018–2020) included 58 patients (41 women and 17 men) with proven H. pylori infection in whom secondary antimicrobial resistance was established. Results: The prevalence of secondary antimicrobial resistance of H. pylori in KBC Split from 2018 to 2020 to certain antibiotics coincides with numerous studies conducted in Europe and Croatia, i.e. the highest resistance was recorded to clarithromycin and levofloxacin. Secondary antimicrobial resistance of H. pylori in KBC Split from 2018 to 2020 was significantly higher in women than in men (p=0,0022). The ratio was 71% to 21%. The highest frequency of secondary antimicrobial resistance of H. pylori with respect to age was recorded in the age group of 60 to 85 years, and was 50% (p<0,0001). Highest antimicrobial resistance was noted with claritromycin, wharas no restance was noted for amoxiciline (p<0,0001). Older age seems to be a protective factor when it comes to resistance on tetracycline (p=0,0013). Conclusion: Research on the secondary resistance of H. pylori to common antibiotics in KBC Split in the three-year period (2018 to 2020) confirms the justification of the treatment strategy based on culture results because after previous failed antimicrobial (empirical) therapies, only targeted antimicrobial therapy leads to eradication of H. pylori.
