Abstract | Ciljevi : Glavni cilj je analizirati morbiditete pacijenata koji dolaze na Hitni neurološki prijem KBC-a Split tijekom potpunog zatvaranja 2020. godine uz usporedbu s istim razdobljem 2019. godine. Cilj je napraviti usporednu analizu najčešćih radnih dijagnoza i medicinske obrade.
Materijali i metode: Provedeno je retrospektivno opažajno istraživanje. Korišteni podatci su prikupljeni iz protokolne knjige Hitnog neurološkog prijema Kliničkog bolničkog centra Split za razdoblja od 18.03. do 03.05. 2019. i 2020. godine. Analiza podataka je rađena u programima STATISTICA 12, Microsoft Word i Microsoft Excel.
Rezultati: U odnosu na razdoblje potpunog zatvaranja 2020. godine, broj pacijenata u istom razdoblju 2019. godine bio je 2,44 puta veći (ꭓ2=275,11; P<0,001). Srednja životna dob pregledanih pacijenata u COVID razdoblju je za 3 godine veća u odnosu godinu ranije (Z=2,48; P=0,013). Muškarci su 1,19 puta učestalije napravili pregled u COVID razdoblju u odnosu na žene (ꭓ2=10,20; P=0,001). Ponedjeljkom je došlo do pada broja pregleda (P=0,047), dok je broj porastao subotom (P=0,029) i nedjeljom (P=0,008). Broj pacijenata upućenih na ambulantnu obradu je 1,23 puta veći u „normalnom“ razdoblju u odnosu na COVID razdoblje (ꭓ2=34,37; P<0,001). Zastupljenost pregleda radi simptoma, znakova i abnormalnih kliničkih, laboratorijskih i neklasificiranih nalaza je smanjena za 11,86% (P<0,001). Udio pregleda radi bolesti mišićno-koštanog sustava i vezivnog tkiva se smanjio za 10,35% (P<0,001). U pacijenata s bolestima krvožilnog sustava utvrđen je porast u broju pregleda za 13,29% (P<0,001). U COVID razdoblju povećao se broj pacijenata s cerebralnim infarktom za 12,94% (P<0,001), a broj pacijenata s dorzalgijom se smanjio za 8,12% (P<0,001).
Zaključci: U razdoblju potpunog zatvaranja 2020. godine, najčešći morbiditeti su bili vezani uz krvožilne bolesti. Bilježi se relativan porast u broju dijagnoza moždanih udara u odnosu na isto razdoblje 2019. godine. Veliki udio pacijenata je upućen na daljnju ambulantnu obradu tijekom lockdowna (62,28%) što ukazuje na veliku opterećenost HNP-a pacijentima koji ne zahtijevaju hitan pregled i obradu. |
Abstract (english) | Objectives: The main objective is to analyze the morbidities of patients coming to the Emergency Neurological Department (END) at the University Hospital of Split during lockdown in 2020 compared to the same period in 2019. The aim is to make a comparative analysis of the most common diagnoses and medical decisions.
Materials and methods: A retrospective observational study was conducted. The data used was collected from the protocol book of the END at the University Hospital of Split for the period from 18.03. to 03.05. 2019 and 2020. Data analysis was performed in STATISTICA 12, Microsoft Word and Microsoft Excel.
Results: Compared to lockdown in 2020, the number of patients in the same period in 2019 was 2.44 times higher (ꭓ2 = 275.11; p <0.001). The mean life expectancy of the examined patients in the COVID period was 3 years higher than in the previous year (Z = 2.48; p = 0.013). Men were examined 1.19 times more frequently in the COVID period than women (ꭓ2 = 10.20; p = 0.001). There was a decrease in the number of examinations on Mondays (p = 0.047), but it was increased on Saturdays (p = 0.029) and on Sundays (p = 0.008). The number of patients referred for outpatient treatment was 1.23 times higher in the “normal” period compared to the COVID period (ꭓ2 = 34.37; p <0.001). The prevalence of examinations for symptoms, signs and abnormal clinical, laboratory and unclassified findings decreased by 11.86% (p <0.001). The share of examinations due to diseases of the musculoskeletal system and connective tissue decreased by 10.35% (p <0.001). In diseases of the vascular system, there was an increase in the number of examinations by 13.29% (p <0.001). In the COVID period, the number of patients with cerebral infarction increased by 12.94% (p <0.001), and the number of patients with dorsalgia decreased by 8.12% (p <0.001).
Conclusions: In the lockdown period of 2020, the most common morbidities were related to vascular diseases. There is a relative increase in the number of stroke diagnoses compared to the same period in 2019. A large proportion of patients were referred for further outpatient treatment during lockdown (62.28%), indicating a high burden of HNP in patients who do not require urgent examination and treatment. |
