Abstract
Ciljevi istraživanja: Glavni cilj ovog istraživanja je utvrditi pojavnost Helicobacter pylori infekcije u bolesnika s upalnim bolestima crijeva. Sporedni ciljevi ovog istraživanja su utvrditi razlike pojavnosti infekcije između bolesnika s Crohnovom bolesti i bolesnika s ulceroznim kolitisom te istražiti postoji li razlika među bolesnicima liječenih konvencionalnom terapijom ili biološkim lijekovima. Ispitanici i postupci: Istraživanjem je obuhvaćeno 64 ispitanika s dijagnozom upalne bolesti crijeva (29 oboljelih od Crohnove bolesti i 35 od ulceroznog kolitisa). 44 ispitanika uzima biološku terapiju, dok je na konvencionalnoj terapiji 20 ispitanika. Svim ispitanicima retrospektivno je pregledana medicinska dokumentacija koja uključuje anamnestičke i demografske podatke te rezultate laboratorijskog testa prisutnosti antigena H. pylori u stolici (imunokromatografski test temeljen na monoklonskim protutijelima). Rezultati: Utvrđena je statistički značajna razlika u prevalenciji H. pylori infekcije bolesnika obuhvaćenih ovim istraživanjem i opće populacije (P<0,001). Nije utvrđena statistički značajna razlika u prevalenciji H. pylori infekcije između bolesnika s Crohnovom bolesti i bolesnika s ulceroznim kolitisom (P=0,843). Bolesnici s upalnim bolestima crijeva liječeni konvencionalnom terapijom temeljenom na aminosalicilatima imaju statistički značajno nižu prevalenciju infekcije u odnosu na one liječene biološkim lijekovima (P<0,001). Između pojedinih skupina bioloških lijekova nije utvrđena statistički značajna razlika (P=0,491). Porastom dobi ispitanika uočen je rast i stope infekcije s Helicobacter pylori (P=0,027). Zaključci: Prevalencija H. pylori infekcije među bolesnicima oboljelim od upalnih bolesti crijeva značajno je niža u odnosu na opću populaciju, ali nije utvrđena razlika između bolesnika s Crohnovom bolesti i bolesnika s ulceroznim kolitisom. Prevalencija H. pylori infekcije značajno je viša kod bolesnika liječenih biološkim lijekovima u usporedbi s onima na konvencionalnoj terapiji temeljenoj na aminosalicilatima, međutim nije utvrđena statistički značajna razlika među pojedinim skupinama biološkim lijekovima. Porastom dobi ispitanika uočen je rast i stope infekcije, a statistički značajna razlika u prevalenciji infekcije H. pylori kod bolesnika s IBD-om ovisna o spolu i pušačkom statusu nije utvrđena.
Abstract (english)
Objectives: The objective of this research is to determine the prevalence of Helicobacter pylori infection in patients with inflammatory bowel diseases. The secondary objectives of this research are to determine the differences in the prevalence of Helicobacter pylori infection between patients with Crohn's disease and patients with ulcerative colitis, and to investigate whether there is a difference between patients treated with conventional therapy or biological drugs. Subjects and methods: A statistically significant difference was found in the prevalence of H. pylori infection between the patients included in this study and the general population (P<0.001). No statistically significant difference was found in the prevalence of H. pylori infection between patients with Crohn's disease and patients with ulcerative colitis (P=0.843). Patients with inflammatory bowel diseases treated with conventional therapy based on aminosalicylates have a statistically significantly lower prevalence of infection compared to those treated with biological drugs (P<0.001). No statistically significant difference was found between individual groups of biological drugs (P=0.491). An increase in the infection rate with Helicobacter pylori was observed as the age of the subjects increased (P=0.027). Conclusions: The prevalence of H. pylori infection among patients suffering from inflammatory bowel diseases is significantly lower compared to the general population, but no difference was found between patients with Crohn's disease and patients with ulcerative colitis. The prevalence of H. pylori infection is significantly higher in patients treated with biological drugs compared to those on conventional therapy based on aminosalicylates, however, no statistically significant difference was found between individual groups of biological drugs. An increase in the infection rate was observed as the age of the subjects increased, and a statistically significant difference in the prevalence of H. pylori infection in patients with IBD depending on gender and smoking status was not established.
