Abstract Cilj istraživanja: Cilj ovog diplomskog rada je osvrt na izdvojene recenzirane studije koje donose rezultate o učinkovitosti, uspješnosti i isplativosti provedenih farmakoekonomskih analiza ljekarničkih usluga. Kontekst ovog istraživanja su ljekarničke usluge u Hrvatskoj koje još uvijek nisu vrednovane, ali postoji potreba da se troškovi pružanja ljekarničkih usluga kvantificiraju.
Ispitanici i metode: Za izradu ovog diplomskog rada pretraživanje literature je provedeno od 1. lipnja 2018. do 1. listopada 2018. godine. Pretraživana je baza MEDLINE (PubMed), pomoću ključnih riječi: „Community pharmacy services‟, „Economic evaluation‟ i „Health economic‟. Uključene su studije koje su objavljenje u periodu od 1. siječnja. 2012. do 31. prosinca 2017. godine. Temeljem kriterija uključenja odlučilo se o njihovom uključenju u istraživanje. Članci su isključeni zbog sljedećih razloga: članci su duplicirani, nije dostupan cjeloviti tekst članka, ljekarničke usluge nisu pružane u ljekarni nego u drugim zdravstvenim ustanovama, učinkovitost ljekarničke usluge nije analizirana u ekonomskim okvirima, provedene su ekonomske procjene lijekova, a ne ljekarničkih usluga, ishodi studija nisu kvantificrani. Pregledni članci, stučna mišljenja ili tzv. editorijali također nisu uključeni.
Rezultati: Izdvojeno je šest relevantnih studija. Tri su studije provedene analizom troška i probitka, dvije studije analizom troška i učinkovitosti, a jedna analizom troška i koristi. Niti jedna analiza minimizacije troškova nije pronađena. Ljekarničke su usluge uključivale: pregled terapije i follow-up susrete, poticanje suradljivosti, prepoznavanje farmakoterapijskih problema, savjetovanje, promicanje zdravih navika, prevenciju bolesti, kontrolna i probirna mjerenja i edukacije pacijenata o terapiji za kronične bolesti.
Zaključci: Farmakoekonomske analize u svijetu i u Hrvatskoj razvijaju se s ciljem optimizacije troškova u zdravstvu te postaju predmet strategije zdravstva nacionalnih sustava. U Hrvatskoj je već otpočela primjena ljekarničkih usluga no farmakoekonmske analize usluga nisu objavljene.
Abstract (english) The aim of the research: The aim of this graduated thesis was to review selected studies that give results on effectiveness, efficiency and cost effectiveness in conducted pharmacoeconomic analysis of community pharmacy services. The context of this research are pharmacy services in Croatia that are still not evaluated, but there is a need to quantify the cost of providing pharmacy services.
Patients and meethods: For the purpose of this graduate thesis the search literature was undertaken between June 1, 2018, and October 1, 2018. The MEDLINE (PubMed) database was searched, using the keywords „Community pharmacy services‟, „Economic evaluation‟ and „Health economic‟. Articles published in English between 2012 and 2017 were included. The databases searched Studies that were selected on the basis of study inclusion criteria. Articles were excluded with the following reasion: aricles were duplicated, a full text was not accessible, the community setting were not practiced in Community Pharmacies but in different Health care settings, evaluating pharmacy intervention effectivness but without conducting a economic evaluationa and economic studies conducted on pharmaceuticals. Review articles and Editorials were also not included.
Results: The 6 remainig studies in this Gratuate theisis undertook economic evaluation of community pharmacy services. Three out oft six studies utilized Cost-Utility Analyses (CUA), two utilized (Cost-Effectivness Analyses (CEA) and one utilized Cost-Benefit Analysis (CBA). No study based on Cost-Minimalization Analysis (CMA) was found in this resarch. Pharmacy services included: Medication review with follow-up (MRF) services, services to increase patient adherence, monitor therapy outcomes, identifying drug-related programmes, counselling, health promotion, prevention, services that provide screening programs and educations of specific chronic disease.
Conclusion: Pharmacoeconomic analyzes in the World and in Croatia are being developed to optimize costs in health care and become the subject of national healthcare strategy. The use of Community pharmacy services has already begun in Croatia, but economic evaluation has not been published yet.
