Objectives: to determine causes of neonatal death according to PSANZ (Perinatal Society of Australia and New Zealand) classification system and to futher divide them based on the gender and early and late neonatal death. Likewise the most common pathological lesions of placenta were noted. Materials and methods: retrospective study over a time span of 7 years evaluated 137 neonatal deaths. The collected data included pathological data, both autopsy reports and placental pathological reports and clinical information from the autopsy request forms. All of the neonatal deaths cases were allocated into one of the PSANZ category and subcategory. Results: Most neonatal deaths occur in the first 7 days (76.7%), specially during first 24 hours (55.2%). The most common cause of death in both groups PDC (Perinatal Death Classification system) and NDC (Neonatal Death Classification system) was perinatal infection (36.2%). The most common neonatal infection was perinatal bacterial pneumonia (69.8%). In patohistological findings the most common lesions were chorioamnionitis and funiculitis (25.9%). According to PSANZ classification there was no statistically significant difference in the cause of death between early and late neonatal period. Conclusion: Perinatal infection was the most common cause of neonatal deaths in PDC and NDC category. Future studies are required to determine reasons behind perinatal infection in order to reduce them and subsequently decrease neonatal death. In order to achive this goal constant cooperation between clinicans and pathologist is of the most importance.
