Abstract
Cilj: Utvrditi učestalost uporabe teleskopski retiniranih, djelomičnih zubnih nadomjestaka u Splitsko-dalmatinskoj županiji u odnosu na druge vrste proteza i na druge u zbrinjavanju nam djelomično bezubog zubala na raspolaganju stojeće metode i tehnike restaurativne protetike. Materijal i metode: Podatke smo prikupljali iz kartotečnih zapisa iz zdravstvenih kartona pacijenata u ordinacijama dentalne medicine Doma zdravlja Splitsko-dalmatinske županije i onih u koncesiji i privatnika. Prikupljeni su podatci potom statistički obrađeni i znanstveno vrednovani. Rezultati: Kod 239 smo pacijenata ustanovili uporabu na teleskopskim krunicama retiniranih djelomičnih zubnih nadomjestaka u petodogišnjem razdobolju (2014. – 2019. god.), 14%. Bilo je 109 pacijenata ženskog spola i 130 pacijenata muškog spola. U gornjoj smo čeljusti ustanovili postojanje 450 zuba nosača (sidara), u donjoj čeljusti 353 sidra, ukupno 803. U dobnoj je skupini između 50. i 60. godine dokazan najveći broj na teleskopskim krunicama retiniranih mobilnih djelomičnih zubnih nadomjestaka u oba spola. Prvi su pretkutnjaci bili najčešće izabrani i upotrijebljeni kao sidra za primarne krunice u oba spola. Svi su pacijenti svojem terapeutu naveli visoki stupanj zadovoljstva pri uporabi ovakve vrste proteze, a naročito zbog mogućnosti provođenja higijene, zbog nepostojanja kvačica kao i zbog slobodnog nepca. Zaključak: kod bolesnika kod kojih su bili provedeni restaurativni protetički zahvati ustanovili smo da uporaba na teleskopskim krunicama retiniranih djelomičnih zubnih nadomjestaka u Splitsko-dalmatinskoj županiji nije ni približna očekivanoj te da su druge vrste proteza predominantne, osobito one s kvačicama. Objektivni su razlozi za spomenutu raspodjelu vjerojatno brojni, a najznačajniji su visoki troškovi. Moguća nešto malo manja tehnološka razvijenost i ne ovladavanje vještinama izrade teleskopskih krunica tijekom izobrazbe zubnih tehničara mogu biti jedan od dodatnih razloga. Kako kod ove vrste zubnih nadomjestaka postoji viši stupanj kvalitete života (QoL) nego li kod drugih vrsta te kako su one tehnološki drugima odmakle daleko naprijed, svaki će napor da se njihova uporaba proširi (ako je to moguće) biti hvalevrijedan.
Abstract (english)
Objectives: To investigate retrospectively the frequency of usage of the telescopic dentures in our population in comparison to other sorts. Patients and Methods: We collected data by analysing individual patient health file in the outpatient dental doctor offices in Split-Dalmatian county. Data were statisticaly processed and scientifically evaluated. Results: In a five year period (2014. – 2019. god.) there were 239 patients that were managed with telescopic crowns as a retention mechanism, 14%. There were 109 female and 130 male patients. There were 450 abutments in the upper jaw and 353 abutments in the lower jaw, in total 803. Most patients that were using telescopic dentures were in the 50 and the 60 year old age group of both sexes. The most fequently used tooth as abutment for the retention of telescopic dentures were first premolars in both sexes. All the patients declared to their dentist a high grade of satisfaction with mentioned dentures because of their practicality, easy higyene andgreat estetics. Conclusion: Among patients who in Split-Dalmatian county in outpatients dental doctor offices underwent restorative prosthodonthics treatment, between those who were examined, telescopic crowns mounted removable partial dentures were, unfortunately, more rarely used than were the other sorts especially those ones with clasps. Plausible reasons for the mentioned distribution are numerous, the most significant one reflected in rather high costs of the construct. Possible less technological development and not acquired skills during professsional education of tooth-laboratory-tehnicians could in some extent be also the explanation. Because this kind of dentures enables higher QoL grade than the other sorts and respecting the fact that they are by far tehnologicaly the most advanced ones, every single effort should be made (if possible) to spread the usage of them widely.
