Abstract
Ciljevi: Ispitati učestalost različitih kategorija preporuka u Cochraneovim sustavnim pregledima za poboljšanje budućih istraživanja iz područja anestezije i boli, uključujući analizu u različitim vremenskim razdobljima te s obzirom na vrstu intervencije istraživanja. Metode: Provedeno je metodološko istraživanje. U istraživanje su uključeni svi aktivni Cochraneovi sustavni pregledi na temu anestezije i boli objavljeni od Cochraneove skupine za anesteziju i Cochraneove skupine za bol i palijativnu skrb. Pretraživanje Cochraneove knjižnice provedeno je 17. srpnja 2020. Preporuke za poboljšanje budućih istraživanja koje se nalaze u odjeljku Implikacije za istraživanje kategorizirane su u 32 kategorije preporuke. Rezultati: U istraživanje je uključeno 370 Cochraneovih sustavnih pregleda, od kojih je 135 (36%) iz Cochraneove skupine za anesteziju, a 235 (64%) iz Cochraneove skupine za bol i palijativnu skrb. Najučestalije kategorije preporuka, zastupljene u preko 40% sustavnih pregleda, u obje skupine, su preporuke za veću veličinu uzorka; bolji odabir ishoda; bolji ustroj istraživanja i bolji odabir intervencije. U preko 20% svih sustavnih pregleda preporučuje se i bolje izvještavanje istraživanja, istraživanje štetnih događaja, odnosno sigurnosti, potreba za dodatnim istraživanjima te bolji odabir kriterija uključenja. Kod svih pet najčešćih kategorija preporuka analiziranih u Cochraneovoj skupini za bol i palijativnu skrb vidljivo je povećanje zastupljenosti u promatranom vremenskom razdoblju. Preporuka za veću veličinu uzorka je jedina kategorija preporuke u Cochraneovoj skupini za anesteziju koja bilježi povećanje zastupljenosti u sustavnim pregledima kroz promatrano vremensko razdoblje. Zaključak: Najučestalije kategorije preporuka su preporuke za veću veličinu uzorka, bolji odabir ishoda, bolji ustroj istraživanja, i bolji odabir intervencije. Kod svih preporuka analiziranih u Cochraneovoj skupini za bol i palijativnu skrb vidljivo je povećanje učestalosti tijekom vremena. Ove preporuke bi trebali koristiti istraživači kako bi ustrojili bolja istraživanja u budućnosti.
Abstract (english)
Objectives: We aimed to analyse recommendations published in the “Implications for research” section of Cochrane reviews for improvements of future trials in the field of anaesthesia and pain, including analyses in different periods and for different types of interventions. Materials and methods: We conducted a methodological study. All currently active Cochrane reviews from the fields of anaesthesia and pain, published by two Cochrane review groups, Cochrane Anaesthesia Group and Cochrane Pain and Palliative Care group were included. A literature search was conducted on July 17, 2020. Recommendations for improvement of future trials were categorized in 32 categories of recommendation. Results: In total, 370 reviews were found eligible for our study, out of which 135 (36%) were published in the Cochrane Anaesthesia Group, and 235 (64%) were published in the Cochrane Pain and Palliative Care group. Larger size sample, better choice of outcome, better design and better choice of future intervention were found to be the most frequent categories of recommendation in both review groups, each present in more than 40% of eligible reviews. Recommendations for better reporting, adverse events and safety, need for additional trials and better eligibility criteria were present in more than 20% of eligible reviews. All five of the most frequent categories of recommendations in the Cochrane Pain and Palliative Care group noted an increase in frequency over the years. Recommendation for a larger size sample was the only common category of recommendation in Cochrane Anaesthesia Group that also noted an increase in frequency. Conclusions: The most frequent categories of recommendations for improvement of future trials are larger size sample, better outcome choice, better study design and better choice of future intervention. All of the most frequent categories of recommendations in the Cochrane Pain and Palliative Care group had increased in frequency over the years. These recommendations should be used by trialists to design better primary studies in the future.
