Abstract Cilj istraživanja: Cilj ovog istraživanja jest usporediti upalne biljege i kliničku sliku bolesnika sa SARS-CoV-2 infekcijom koji su preminuli i bolesnika sa SARS-CoV-2 infekcijom koji su preživjeli.
Ispitanici i metode: U ovo retrospektivno istraživanje uključeno je 219 SARS-CoV-2 pozitivnih bolesnika koji su u periodu od veljače do srpnja 2020.godine hospitalizirani u Kliničkom bolničkom centru Split. Ispitanici su podijeljeni u dvije skupine. U prvu skupinu su uključeni podatci od 177
... More ispitanika koji su preživjeli, a u drugu skupinu podatci od 42 ispitanika koji su preminuli. Svi su podatci prikupljeni iz elektronske baze podataka Kliničkog bolničkog centra Split.
Rezultati: Pronađena je statistički značajna razlika u životnoj dobi između skupine bolesnika koji su preživjeli i skupine bolesnika koji su preminuli (74 vs. 84,5; P<0,001). Hipertenzija je najčešći komorbiditet u obje skupine, a u skupini bolesnika koji su preminuli bila je značajno češće zastupljena od skupine koja je preživjela (62% vs. 55%; P=0,009). Udio preostalih komorbiditeta bio je statistički značajno veći u skupini bolesnika sa SARS-CoV-2 infekcijom koji su preminuli, osim malignoma koji su statistički značajno češći bili u skupini bolesnika sa SARS-CoV-2 infekcijom koji su preživjeli. Statistički značajno povišene vrijednosti CRP-a i prokalcitonina, a snižena vrijednost albumina pronađene su u bolesnika sa SARS-CoV-2 infekcijom koji su preminuli (P<0,001). Vrijednosti D-dimera i fibrinogena statistički su značajno više u skupini bolesnika sa SARS-CoV-2 infekcijom koji su preživjeli (P=0,002; P=0,035). Statistički značajna razlika pronađena je između skupine bolesnika sa SARS-CoV-2 infekcijom koji su preživjeli i skupine bolesnika koji su preminuli u omjeru neutrofila i limfocita (P=0,028), limfocita i CRP-a (P=0,004) te CRP-a i albumina (P<0,001). Skupina bolesnika sa SARS-CoV-2 infekcijom koji su preživjeli, u usporedbi sa skupinom bolesnika koji su preminuli, imala je značajno manji postotak radiološki opisane obostrane upale pluća (38% vs. 59%; P<0,001). Statistički je značajno veća potreba za invazivnom mehaničkom ventilacijom bila u skupini bolesnika sa SARS-CoV-2 infekcijom koji su preminuli (50% vs. 7% ; P<0,001).
Zaključak: Bolesnici sa SARS-CoV-2 infekcijom koji su preminuli su bili starije životne dobi, s većim udjelom komorbiditeta, značajno većim vrijednostima laboratorijskih pokazatelja upalne aktivnosti, značajno većom učestalosti obostrane upale pluća i potrebe za mehaničkom ventilacijom u usporedbi s bolesnicima sa SARS-CoV-2 infekcijom koji su preživjeli. Abstract (english) Diploma thesis title: Relationship between inflammatory markers and clinical manifestation in patients with SARS-CoV-2 infection
Aim of the study: The aim of this study was to compare inflammatory markers and the clinical manifestations of patients with SARS-CoV-2 infection who died and patients with SARS-CoV-2 infection who survived.
Subjects and methods: This retrospective study included 219 SARS-CoV-2 positive patients who were hospitalized in the University Hospital Split in the
... More period from February to July 2020. Respondents were divided into two groups. The first group included data from 177 survivors, and the second group included data from 42 survivors. All data were collected from the electronic database of the University Hospital Split.
Results: A statistically significant difference in age was found between the group of surviving patients and the group of deceased patients (74 vs. 84.5; P <0.001). Hypertension was the most common comorbidity in both groups and was significantly more common in the deceased group than in the surviving group (62% vs. 55%; P = 0.009). The proportion of other comorbidities was statistically significantly higher in the group of patients with SARS-CoV-2 infection who died, except for malignancies that were statistically significantly more common in the group of patients with SARS-CoV-2 infection who survived. Statistically significantly elevated CRP and procalcitonin values and decreased albumin values were found in patients with SARS-CoV-2 infection who died (P <0.001). D-dimer and fibrinogen values were statistically significantly higher in the group of patients with SARS-CoV-2 infection who survived (P = 0.002; P = 0.035). A statistically significant difference was found between the group of patients with SARS-CoV-2 infection who survived and the group of patients who died in the ratio of neutrophils and lymphocytes (P = 0.028), lymphocytes and CRP (P = 0.004) and CRP and albumin (P <0.001). The group of patients with SARS-CoV-2 infection who survived, compared with the group of patients who died, had a significantly lower percentage of radiologically described bilateral pneumonia (38% vs. 59%; P <0.001). There was a statistically significantly higher need for invasive mechanical ventilation in the group of patients with SARS-CoV-2 infection who died (50% vs. 7%; P <0.001).
Conclusion: Patients with SARS-CoV-2 infection who died were elderly, with a higher proportion of comorbidities, significantly higher values of laboratory indicators of inflammatory activity, significantly higher incidence of bilateral pneumonia and the need for mechanical ventilation compared with patients with SARS- CoV-2 infection survivors.
