Abstract Cilj: Napraviti pregled podataka o bolesnicima otkrivenim Nacionalnim programom za probir i rano otkrivanje raka pluća koji su dijagnosticirani i operirani u Kliničkom bolničkom centru Split. Ispitanici i metode: U istraživanje su uključeni nalazi 22 bolesnika koji su dijagnosticirani putem Nacionalnog programa za probir i rano otkrivanje raka pluća u KBC-u Split u periodu od 2020. do 2024. godine. Operirani bolesnici su detaljno obrađeni korištenjem dostupne bolničke dokumentacije. Pritom su analizirani osnovni demografski podaci (dob i spol), vrijeme proteklo od dijagnoze do operacije, vremensko trajanje hospitalizacije, tip operativnog zahvata, histološki tip karcinoma te pušački staž bolesnika. Za provođenje ovog istraživanja pretraživan je Bolnički informacijski sustav KBC-a Split u razdoblju od 1. siječnja 2020. do 31. prosinca 2023. Rezultati: Od ukupnog broja obrađenih bolesnika 15 je muškog spola (68%), dok je 7 ženskog spola (32%). N ajviše ih je bolovalo od adenokarcinoma pluća (9 bolesnika – 41%), zatim od planocelularnog karcinoma (6 bolesnika – 27%), dok je sitnostanični karcinom potvrđen kod dva bolesnika (9%). U trenutku postavljanja dijagnoze najmlađi bolesnik imao je 51 godinu, najstariji 73 godine, a medijan i srednja vrijednost iznose 65 godina. Od postavljanja sumnje opisivanjem lezije na računalnoj tomografiji niske doze zračenja do operacije najkraće je prošlo 2 mjeseca, a prosječno vrijeme do operativnog zahvata iznosilo je 4 mjeseca. Najviše bolesnika otkriveno je probirom dok je bolest bila u u ranijim stadijima, 11 bolesnika u prvom stadiju (50%), a njih petero u drugom stadiju (22%). Uz kirurško liječenje, 18 bolesnika zahtjevalo je neki oblik onkološkog liječenja. 16 bolesnika podvrgnuto je adjuvantnon kemoterapijskom liječenju (72%), a njih 18% nakon operacije bilo je bez potrebe za dodatnim modalitetima liječenja. Zaključci: Većina bolesnika otkrivena putem Nacionalnog programa za probir i rano otkrivanje raka pluća u KBC-u Split su muškarci, a prosječna dob bolesnika je 65 godina. Najučestaliji karcinom operiran u KBC-u Split je adenokarcinom pluća, a zatim planocelularni karcinom. Kandidati za kirurško liječenje se otkrivaju putem Nacionalnog programa za probir i rano otkrivanje raka pluća u ranijim kliničkim stadijima bolesti (I. i II.). Najčešći kirurški pristup u novootkrivenih bolesnika s karcinomom pluća jesu video asistirane torakoskopske lobarne ili sublobarne resekcije. Većina novootkrivenih i kirurški liječenih bolesnika zahtijeva neki vid onkološkog liječenja.
Abstract (english) Research objective : To review data of patients detected by the National Program for Screening and Early Detection of Lung Cancer who were diagnosed at University Hospital of Split and underwent surgery. Subjects and methods : The research included the findings of 22 patients who were diagnosed through the National Program for Screening and Early Detection of Lung Cancer at University Hospital of Split in the period from 2020 to 2024. The operated patients were processed in detail using the available hospital documentation. Basic demographic data (age and gender), time elapsed from diagnosis to surgery, duration of hospitalization, type of surgery, histological type of cancer and smoking history of the patient were analyzed. Hospital Information System was searched in the period from January 1, 2020 to December 31, 2023. Results : Of the total number of treated patients, 68% were male, while 32% were female. Most of them suffered from lung adenocarcinoma (41%), followed by squamous cell carcinoma (27%). At the time of diagnosis, the youngest patient was 51 years old, the oldest 73 years old, and the median and mean values are 65 years old. From raising suspicion by describing the lesion on low-dose computed tomography to surgery, the shortest time elapsed was 2 months, and the average time to surgery was 4 months. Most patients were detected by screening while the disease was in the earlier stages, 50% of patients in the first stage, and 22% of them in the second stage. In addition to surgical treatment, 18 patients required some form of oncological treatment. 16 patients underwent adjuvant chemotherapy treatment (72%), and 18% of them did not need additional treatment modalities after surgery. Conclusions: The majority of patients detected through the National Program for Screening and Early Detection of Lung Cancer at University hospital of Split are men, and the average age of patients is 65 years. The most common operated type of cancer is lung adenocarcinoma, followed by squamous cell carcinoma. Candidates for surgical treatment are detected through the National Program for screening and early detection of lung cancer in the earlier clinical stages of the disease (I. and II.). The most common surgical approach in newly diagnosed patients with lung cancer is video-assisted thoracoscopic lobar or sublobar resection. The majority of newly diagnosed and surgically treated patients require some form of oncological treatment.
