Abstract
NASLOV RADA: Utjecaj spola u poremećajima srčanoga ritma koji su bili indikacija za ugradnju trajnoga srčanoga elektrostimulaora. CILJ ISTRAŽIVANJA: Cilj diplomskog rada je utvrditi postojanje spolnih razlika u poremećajima srčanoga ritma koji su bili uzrok ugradnje trajnoga srčanoga elektrostimulatora. ISPITANICI I METODE: Studija je organizirana kao retrospektivno kohortno istraživanje. U radu su korišteni podatci iz povijesti bolesti i druge dostupne medicinske dokumentacije Klinike za unutarnje bolesti, KBC-a Split, u vremenskom periodu od 2001. godine do 2010. godine. Glavni ulazni podatci su spol ispitanika i poremećaji srčanoga ritma koji su bili indikacija za ugradnju trajnoga srčanoga elektrostimulatora. Prilikom odabira pacijenata jedini kriterij uključenja bila je prvougradnja trajnoga srčanoga elektrostimulatora. Glavne mjere ishoda su pojedine vrste srčanih bradiaritmija koje su uzrok postavljanja trajnoga srčanoga elektrostimulatora. Usporedili smo učestalost tih aritmija u muškaraca i žena, a promatrane parametre izrazili smo u postotcima. REZULTATI: Ukupno je bilo 2263 ispitanika kojima je ugrađen trajni srčani elektrostimulator. Od ukupnoga broja ispitanika bilo je 1400 (62%) muškarac i 863 (38%) žene. Rezultati analize ukazuju da su se zbog fibrilacije atrija (P=2,6x10-6), bolesti sinusnoga čvora (P=0,016) i atrioventrikularnoga bloka III. stupnja (P=0,045) trajni srčani elektrostimulatori statistički značajno češće ugrađivali prema spolu, a za ukupni atrioventrikularni blok (P=0,065), atrioventrikularni blok II. stupnja (P=0,59) i atrioventrikularni blok II/III. stupnja (P=0,41) prema spolu nismo utvrdili statističku značajnost. ZAKLJUČCI: Provedena analize dokazuje da je fibrilacija atrija statistički značajno češća indikacija za ugradnju trajnoga srčanoga elektrostimulatora u muškarca, bolest sinusnoga čvora statistički značajno češća indikacija za ugradnju u žena, a atrioventrikularni blok III. stupnja bio je statistički značajno češća indikacija za ugradnju trajnoga srčanoga elektrostimulatora u muškaraca. Ukupni atrioventrikularni blok, atrioventrikularni blok II. stupnja te atrioventrikularni blok II•/III. stupnja nisu pokazali statističku značajnost da se kao indikacije za ugradnju trajnoga srčanoga elektrostimulatora ugrađuju češće prema spolu.
Abstract (english)
RESEARCH AIM: The goal of the thesis is to determine the existence of gender differences in heart rhythm disorders that were the reason of cardiac pacemaker installation. PARTICIPANTS AND METHODS: The study is organized as a retrospective cohort study. The data in the study are used from medical history and other available medical documentation of the Internal Medicine department, Clinical Hospital in Split, in the period from 2001. to 2010. year. The main input data are gender of the respondents and cardiac arrhythmias that were the indication for the installation of permanent cardiac pacemaker. When selecting patients only inclusion criterion was primo-implantation of permanent cardiac pacemaker. The main outcome data were certain types of cardiac bradyarrhythmias that cause permanent installation of cardiac pacemaker. We compared the frequency of arrhythmias within men and women and the observed parameters are expressed in percentages. RESULTS: There were 2263 patients with permanent cardiac pacemaker implanted. There were 1400 (62%) men and 863 (38%) women of the total number of respondents. The results indicate that the atrial fibrillation (P=2,6x10-6), sinus node disease (P = 0.016) and atrioventricular block third grade (P = 0.045) are significantly more frequently installed by gender, and for all atrioventricular blocks (P = 0.065), atrioventricular block second grade (P = 0.59) and atrioventricular block II/III. grade (P=0.41) we did not found statistical significance for permanent pacemaker installation by gender. CONCLUSION: The conducted analysis show that atrial fibrillation was significantly more frequent indication for implantation of permanent pacemakers in male respondents, sinus node disease was significantly more common indications for implantation in women, and atrioventricular block III. grade was significantly more frequent indication for permanent installation of cardiac pacemakers in males. Total atrioventricular block, atrioventricular block II. grade and the atrioventricular block II•/III. grade did not show statistical significance as indications for permanent installation of cardiac pacemaker by gender.
