Abstract
CILJ ISTRAŽIVANJA: Usporediti dobivene podatake izmedu OCT - novije strukturne dijagnostike i VP - funkcionalne dijagnostike koja se ranije isključivo koristila. ISPITANICI I METODE: Istraživanjem je obuhvaćeno 68 očiju u 38 ispitanika od kojih je 12 (32%) bilo muškaraca, a 26 (68%) žena sa klinički manifestnim primarnim glaukomom otvorenog kuta i onih koji su suspektni na glaukom, te okularnu hipertenziju. Medijan životne dobi ispitanika iznosio je 61 godinu (min-maks: 11-80 godina). Rezultati su prikazani aritmetičkim sredinama ±SD ili medijanima (min-maks). U svih ispitanika proveden je cjeloviti oftalmološki pregled, OCT pregled, mjerenje debljine RNFL i pregled vidnog polja. Podatci su prikupljeni na Klinici za očne bolesti, Kliničkog bolničkog centra Split, Medicinskog fakulteta u Splitu. U statističkoj obradi su korišteni: Pearsonov test korelacije, ANOVA s post hoc Bonferroni testom i Kruskal Wallis. REZULTATI: Istraživanjem je obuhvaćeno 68 očiju u 38 ispitanika od kojih je 12 (32%) bilo muškaraca, a 26 (68%) žena. Medijan životne dobi ispitanika iznosio je 61 godinu (min-maks: 11-80 godina). MS ukupnog broja očiju ima pozitivnu korelaciju s prosječnom debljinom RNFL, RNFL superiorno i RNFL inferiorno. MD ukupnog broja očiju pozitivno korelira s prosječnom debljinom RNFL, kao i s RNFL superiorno i RNFL inferiorno. MS ukupnog broja očiju pozitivno korelira s prosječnom debljinom RNFL, kao i s RNFL superiorno i RNFL inferiorno. VISUS korelira s RNFL inferiorno i nazalno. C/d ukupnog broja očiju pozitivno korelira s prosječnom debljinom RNFL, kao i s RNFL superiorno i RNFL inferiorno. Intraokularni tlak pozitivno korelira s RNFL inferiorno. ZAKLJUČAK: Promjene debljine RNFL mjerene OCT-om upućuju na funkcionalna oštećenja koja se otkrivaju pregledom vidnog polja. Prosječna debljina RNFL nije pouzdan parametar za ranu dijagnozu i praćenje glaukoma i očne hipertenzije, dok su promjene po segmentima glave vidnog živca, te mjerenje eskavacije glave vidnog živca pouzdanije naročito superiorno i inferiorno. Određivanje ekskavacije, tj. odnosa promjera ekskavacije i promjera papile vidnog živca, parametar je koji nam ukazuje na stupanj propadanja vidnog živca.
Abstract (croatian)
RESEARCH OBJECTIVE: The aim of this study is to compare the findings of optical coherence tomography and functional changes in the visual fields in patients with ocular hypertension (OH) and primary open angle glaucoma (POAG). MATERIALS AND METHODS: This research included 68 eyes within 38 people from which 12 (32%) was male and 26 (68%) was female with glaucoma or without glaucoma. Median age was 61 (min-max: age 11-80). Results are presented with arithmetic mean ±SD or medians (min-maks). Enrolled patients were put through complete ophtalmological examination, OCT examination, measurement of the thickness of retinal nerve fiber layer (RNFL) andvisualfield. Data for this study was collected and conducted in the Eye Clinic University Hospital Split, School of Medicine Split. In statistical analysis were used: Pearson correlation test, ANOVA with post hoc Bonferroni test and Kruskal Wallis test. RESULTS: The study included 68 eyes within 38 patients from which 12 (32%) male and 26 (68%) female. Median of age was 61 (min-max: age 11-80). MS of total eyes has positive correlation with average thickness of the RNFL, RNFL superior and RNFL inferior. MD of total eyes has positive correlation with average thickness of the RNFL, RNFL superior and RNFL inferior. MS of total eyes has positive correlation with average thickness of the RNFL, RNFL superior and RNFL inferior. VISUS has positive correlation with RNFL inferior and RNFL nasal. C/d of total eyes has positive correlation with average thickness of the RNFL, RNFL superior and RNFL inferior. Intraocular pressure has positive correlation with RNFLinferior. CONCLUSION: Structural changes of RNFL thickness measured with OCT, indicate functional damage discovered by examination of the visual field. Measurement of segmental changes of the RNFL thickness and excavation are more reliable than measurement of the average RNFL thickness in early diagnosis and monitoring of glaucoma. Determining excavation and diameter of the visual nerve head is a parameter which tells us about visual nerve damage.
