Background: Bacteremia that evloves into sepsis is a major global health problem. Its diagnosis and treatment creates enormous expenses and load for the healthcare system. The base of the treatment are empiric antimicrobial drugs whose effectiveness has been shown to be greatest in the case of prompt and appropriate administration. Objectives: The aim of this study is to determine the frequency and the distribution of the most significant bacteremia pathogens isolated in the University Hospital of Split in 2018. and to determine their sensitivity and resistance to antimicrobial drugs. Materials and methods: In this retrospective study, data has been used from the archive of the Department od Microbiology and Parasitology, University Hospital of Split. Data included all pathogen positive blood cultures and the corresponding results of antibiograms that tested susceptibility to antimicrobial drugs in the period from January 1,2018 to December 31, 2018. Repetitive or so-called "Copy" strains were not included in the analysis. Results: A total of 1165 primoisolates were isolated from blood cultures and their susceptibility to antimicrobial drugs was tested. Gram-positive bacteria were isolated more often (66%) than Gram-negative ones. The most commonly isolated Gram-positive bacteria are coagulase-negative staphylococci (28% of all positive blood cultures). The most commonly isolated Gram-negative bacterium is E. coli (10.7%). All staphylococcal isolates are susceptible to vancomycin, regardless of whether they are coagulase negative or coagulase positive strains and regardless of the presence of methicillin resistance. All E. coli isolates were susceptible to imipenem and meropenem, while 7.3% of K. pneumoniae isolates produced carbapenemases (KPCs). A certain proportion of enterobacteria (22.4% of total isolated E. coli and 35.4% of K. pneumoniae isolates) produce extended-spectrum beta-lactamases (ESBL). All Acinetobacter spp. isolates were sensitive to colistin, mostly sensitive to ampicillin / sulbactam, and resistant to other antibiotics tested. The highest resistance of Pseudomonas aeruginosa was to quinolones, and the lowest to amikacin. Conclusion: Susceptibility testing of bacterial isolates from blood culture is an indispensable part of good microbiological practice. Based on the monitoring of sensitivity results over a period of time, effective empirical therapy can be determined, which is corrected according to the results of sensitivity testing.
